    Waiver of Release or Liability - The Escape Spot

    Please Read Below Carefully

    Before Visiting The Escape Spot:

    EVERY MEMBER OF YOUR GROUP MUST COMPLETE AND SIGN THE FORM BELOW.

    For Minor Children (under 18 years of age) an Adult over the age of 18 may sign for them, but everyone intending to participate in The Escape Spot experience much have a completed Waiver form on file.

    • WAIVER OF RELEASE OR LIABILITY FORM FOR THE ESCAPE SPOT
      I, THE UNDERSIGNED, HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THIS ESCAPE ROOM EVENT FOR MYSELF AND ANY MINORS WHOSE NAMES ARE INCLUDED BELOW given by The Escape Spot (“the Company”), including, but not limited to, any risks that may arise from negligence or carelessness on the part of the Company, its employees, and its agents being released, from dangerous or defective equipment or property owned, maintained, or controlled by the Company, or because of the Company’s, or its employees and agents’, possible liability without fault.

    • I certify that I understand this activity has potential risks, including but not limited to:
      - Use of simple tools;
      - Potentially moving or lifting objects of not more than 20 pounds;
      - Mental stress and anxiety;
      - Being in an enclosed space with other persons (doors are not locked for safety); or
      - Not completing task of escaping in allotted time (60 minutes).
    • I certify that:
      - I have no physical or mental illness that precludes my participation in a safe manner for myself or others;
      - I am not under the influence of drugs or alcohol which impairs my ability to maintain my safety awareness or endangers others;
      - I acknowledge that this Waiver of Release or Liability Form will be used by the organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity;
      - I agree that all staff or authorized agents may, in their sole discretion, determine it is unsafe for myself or others for my participation to continue and remove me from the premises by any lawful means; and
      - I acknowledge that I may be held liable for any damages incurred to the props, game elements or structure of the facility due to my negligence or malicious behavior.
    • I certify that I understand these temporary Covid-19 measures apply at all times within the Company’s facilities:
      - Waivers must be signed for by all players prior to entering room, including the names and details of every player needed at time of booking;
      - Players (and staff) are required to wear masks in common spaces;
      - Washing hands/sanitizing thoroughly before and after playing is strongly recommended;
      - There is a maximum of 8 players (room dependent), preferably only your existing social distance group;
      - Room play times have been staggered to limit the number of participants in the building at one time; and
      - If you are sick, or have any symptoms as noted in store signage, you will be asked to reschedule your participation in any event.
    • An adult over 18 years of age may sign the waiver for minor children who are accompanying them to The Escape Spot.
